LIHEAP FY 2013 Plan - The following will be implemented in 2013 for the LIHEAP Program

The Application
The application must be completed before it is accepted at the LIHEAP Office. The application must be in the homeowners/renters name and all household members must be listed. Attach verification of income for; FAIM, SSI, SSA, GA, VA, Retirement, Unemployment, self-employment for all household members, if they are over the age of 18 and not working a form has to be filled out. Social Security numbers for each family member MUST be on the application and a copy of the Tribal ID must be attached. The house number & physical address MUST be on the application. Please check if you have been assisted with weatherization within the past 10 years from Action for Eastern Montana, Glendive, MT., this will determine eligibility for weatherization for your home. When the application is completed you will be notified if you have been approved and the amount you are approved for. We will assist you when we receive our funding.

The Electricity Bill
The electricity bill MUST be in the applicants/renters name, bring in a copy of your electricity bill for the month of September. If renting, an AFFIDAVIT FORM must be filled out by the renter and landlord. If you already filled one out last year you will not have to complete another one.
Primary source of heat: The electricity bill will be paid for the total amount of the award when the funds are received in the finance office.
Secondary source of heat: Electricity - LIHEAP will pay 30% of the electricity bill and the client is responsible for the remaining bill each month.
WAPA is for homeowners who are 60 years and older, living in the home and the bill is in their name. TRECO will notify the LIHEAP Program each month of the amount that will be deducted from your electricity bill.
If the applicant dies during the Fiscal Year from October - March then LIHEAP will assist in changing the bill into the surviving spouse's name ONLY. LIHEAP will not pay for other household members and they will have to submit an application and the bill has to be changed to their name.

Requests for Propane
The total amount of the approved award will be sent to the Vendor when Central Finance receives the funds. The homeowner MUST be the only person to call in for requests for propane. In case the Homeowner is unable to call, a designated person should be listed on the application. The homeowners house number and physical address MUST be on the Fuel Order when we fill it out if you do not know your house number contact Housing Authority. The Homeowner MUST sign the Fuel Order when the tank is filled. You need to make arrangements for someone to be home when the propane tank is filled. LIHEAP will not pay for tank rentals it is the responsibility of the Homeowner. Please put your fuel supplier on the application, if you have a rental the only vendor to put in the propane is who you are renting from. If you own your tank, please put that on the application and who you prefer. Sometimes the vendor is too busy and we have to contact another one. CRISES: You must have an approved application and one-time assistance will be given. Crises will be given after the award amount is zeroed out.

Wood
Bids will go out for WOOD Vendors and we will notify you who is selected. This will go through the Tribal Procurement Office. Do not put wood vendor on you application we will notify you who has been selected. You MUST not sell your wood to anyone after you receive it or you will be cut off for the rest of the year.
